Smartphones Can Now Run Consumers’ Lives

LAS VEGAS — The smartphone is no longer just a portable computer in your pocket. It has become the remote control for your life.
Want to flip off the living room lights, unlock your front door, or get a reading of your blood pressure? All of this can be done through mobile apps that work with accessories embedded with sensors or an Internet connection.
For several years, technology companies have promised the dream of the connected home, the connected body and the connected car. Those connections have proved illusory. But in the last year app-powered accessories have provided the mechanism to actually make the connections. That is partly because smartphones have become the device people never put down. But it is also because wireless sensors have become smaller, cheaper and ubiquitous.
Big companies with strong brands have been heavily promoting the new uses for these gadgets. General Motors advertises its Chevy Malibu Eco with a man showing his parents how he starts the car with a smartphone. A major selling point of the popular Nest thermostat is its ability to turn up the furnace from miles away with a cellphone.
“Now that, increasingly, consumers have a device with them to monitor virtually anything they do with the Internet, why not offer that capability to monitor and remote control?” said Ross Rubin, an analyst at Reticle Research.
The idea of turning off the lights with a smartphone may seem gimmicky, but consumers are warming to applications, said Bill Scheffler, director of business development for the Z-Wave Alliance, a consortium of companies that make connected appliances. The situation resembles the time when power windows started catching on for automobiles, or when television makers started offering remote controls, Mr. Scheffler said.
“It used to be that people would say, why does anybody want a remote control for a TV if you can get up and change the channel?” he said. “It’s just progress.” Companies like AT&T, Black and Decker and Honeywell have started selling app-linked products, he said.
At the International Consumer Electronics Show, which has attracted more than 150,000 people here this week, dozens of companies are showing off connected accessories they can hook up to their home appliances to make them work with smartphones, and many are also displaying wearable devices that can help people monitor their health on their phones. Some of these products are being provided by large companies. AT&T, the wireless carrier, said that in March it will begin selling a wireless security system called Digital Life that will allow people to use tablets or phones to monitor cameras, alarms and even coffeepots.
If a burglar trips a motion sensor in the house, for example, a user can receive a text message, then call the police. Customers can choose to expand AT&T’s wireless service to appliances like lights, door locks, thermostats and security cameras, which can be controlled and monitored through the AT&T mobile app.
Ralph de la Vega, chief executive of AT&T Mobility, said in an interview that home security was a big opportunity to increase revenue. Only 20 percent of homes have security systems, he said, leaving millions of homeowners as potential buyers.
“I think it dramatically changes how people feel about their home and how secure they feel about being outside the home,” Mr. de la Vega said. “I think it’s an easy sell.” The company has not announced prices for the service.
Ingersoll Rand, which makes industrial products, offers a $300 starter kit and software for people to connect their homes. It includes a lock, a light and a wireless “bridge,” or base station, to connect the devices to the Internet. They can be controlled with a smartphone or tablet app called Nexia Home Intelligence. Customers also must pay at least $9 a month for a subscription; they can choose to buy the appliances and the bridge separately.
Products by several other companies take advantage of a smartphone’s sensors and connection to the Internet to monitor consumers’ health. IHealth sells monitors for people to track their blood pressure with an app. At the electronics show, it introduced a wireless glucose meter, called the Smart Glucometer, that lets people with diabetes determine their blood sugar. A user puts a blood sample on a test strip, pops it into an accessory attached to a smartphone, and an app gives a reading of the blood sugar level.
Adam Lin, general manager of iHealth, declined to say how many products the company had sold, but he said it was in the “six-figure” area. IHealth products have appeared at Apple, Target and Best Buy.
In addition to people who are interested in their health, health insurance providers might embrace monitoring products. Mr. Lin said iHealth was discussing with two insurers whether to provide its products to patients, which would help reduce their doctor visits.
A small start-up, AliveCor, has created an iPhone case that when grasped records an accurate electrocardiogram on the iPhone screen via its app. The company has attracted financing from Khosla Ventures, a prominent Silicon Valley venture capital firm.
Nike, Jawbone and Fitbit sell wearable electronic devices for people to track their movements with smartphones. Fitbit, based in San Francisco, sells a pocket pedometer called the Fitbit One, which can track a user’s steps, floors climbed and sleep. Its newest product is due in spring, the Fitbit Flex, a step counter and sleep tracker that is worn around the wrist. It synchronizes with a smartphone app to give users updates.
Woody Scal, chief revenue officer of Fitbit, said the company sells its devices in 10,000 retail stores in the United States. Its Fitbit One is the best-selling sports device on Amazon.com. He said one reason that wearable fitness gadgets have become popular is that the sensors have shrunk and battery life has improved. That helps make the products slimmer, more stylish and easier to use.
Mr. Scal said wireless fitness devices were becoming popular because they address basic needs for consumers, unlike another trend seen at the show, enormous televisions.
“In the end, I don’t wake up in the morning, look myself in the mirror and ask whether my TV has enough pixels,” he said. “But I do wonder how I’m going to get enough exercise, eat better, sleep well, or manage my weight despite all the other things going on in my life.”

WHEN aliens seize and torture Dr McCoy in “The Empath”, an episode of the science-fiction series “Star Trek”, Captain Kirk and Mr Spock rush to his aid. They are able to assess his condition in seconds with the help of a medical tricorder—a hand-held computer with a detachable sensor that is normally used by Dr McCoy himself to diagnose others. A quick scan with the tricorder indicates that he suffers from “severe heart damage; signs of congestion in both lungs; evidence of massive circulatory collapse”.

Along with teleportation, speech-driven computers and hand-held wireless communicators that flip open, the medical tricorder was one of many imaginary future technologies featured in “Star Trek”. Ever since, researchers have dreamed of developing a hand-held medical scanner that can take readings from a patient and then diagnose various conditions. Now, nearly five decades after “Star Trek” made its debut in 1966, the dream is finally edging closer to reality.

Among the organisations pushing for the development of a medical tricorder is the X Prize Foundation, an organisation that aims to spur innovation by offering cash prizes. Earlier this year it announced the Qualcomm Tricorder X Prize, financed by the Qualcomm Foundation, the charitable arm of Qualcomm, a maker of wireless communications technologies. It has put up $10m in prize money and another $10m to pay for the administration of the competition. So far more than 230 teams from over 30 countries have applied to enter the contest, the guidelines for which will be finalised this month. The goal is to create a mobile platform that will enable people to diagnose a set of 15 conditions, including diseases as varied as pneumonia, diabetes and sleep apnoea, without having to rely on a doctor or nurse. “Ultimately this is about democratising access to health care around the world,” says Peter Diamandis, the head of the X Prize Foundation.

But the obstacles to building a medical tricorder are not merely technological. Regulatory agencies such as America’s Food and Drug Administration (FDA) may delay or restrict consumers from getting their hands on such devices, and the medical establishment, infamous for its inertia, may be wary of granting patients a more active role in diagnosis. Many doctors do not believe that patients can be trusted with their own medical data and are reluctant to give them access to it, explains Eric Topol, a cardiologist and the author of “The Creative Destruction of Medicine”. He believes the push to adopt new digital technologies in health care will have to come not from doctors but from the public.

Making self-service diagnostic technology cheaper and more widely available would, however, have enormous benefits in both rich and poor parts of the world. The Association of American Medical Colleges projects that America could have 90,000 doctors fewer than it needs by 2020, as doctors retire, the population ages and chronic illnesses become more prevalent. All this will place huge demands on America’s sprawling health-care system, and threatens to increase health-related spending still further. Other rich countries are also looking for ways to keep a lid on rising health-care expenditure.

Paging the real McCoy

In developing countries, meanwhile, large numbers of people live in rural areas far away from hospitals and medical centres, reducing access to diagnosis and treatment. There are also far fewer doctors per capita: around two doctors per 10,000 people in Africa as a whole in 2010, compared with 33 in Europe. “No country in the world is producing enough doctors and nurses to satisfy their current or future demands for health care,” says Christopher Wasden, an expert in health-care strategy and innovation at PricewaterhouseCoopers (PwC), a consultancy. “Unless we have consumers doing more of this themselves, it’s impossible to deliver care to those people who need it.”

The prospects for creating a medical tricorder have been boosted enormously by the spread of mobile phones. There are now more than 6 billion in use around the world, of which around 1 billion are smartphones—in effect, powerful, internet-capable pocket computers.

Even without any additional hardware or software, a phone can be a useful medical device. Some health-care organisations already send out text messages to patients’ mobile phones, for example, reminding them to take their medicine, renew their prescriptions or visit a doctor. Add some extra software in the form of downloadable apps, and the cameras and video recorders built into more advanced handsets can be used as sensors to measure or track vital signs, such as heart and respiration rates. Add hardware in the form of sensors that plug into the phone or connect to it wirelessly, and a phone can become an even more powerful tool for monitoring and diagnosis.

According to research2guidance, a consultancy, global sales of mobile-health apps will increase from $718m in 2011 to $1.3 billion this year. Many of these might be more accurately described as fitness or “self-tracking” apps that allow people to monitor their exercise regimes, energy consumption and sleep patterns, often in conjunction with external sensors. But PwC’s research suggests that consumers want more. “They want apps that really guide their behaviour and can replace a nurse or a doctor by using intelligent algorithms to guide patients toward healthier habits,” says Mr Wasden. But, he adds, “as you add intelligence to an app, you’ve increased the likelihood that the FDA is going to want to review it.”

At the moment there is only draft guidance from the FDA on which medical apps require review. A review is deemed necessary only for mobile-health apps that are used in conjunction with already regulated medical devices, or that would transform a mobile device, such as a smartphone or tablet, into a regulated medical device, typically in conjunction with sensors or attachments that help with diagnosis or treatment. The FDA says its final guidelines will be available by the end of the year. In addition, it intends to regulate clinical decision-support software, which helps make diagnoses or recommends treatments. This will be tackled separately from mobile medical apps, says Bakul Patel, a policy adviser at the FDA. Draft guidance should be available in the coming months, he says.

Mr Patel says the FDA wants to encourage innovation while ensuring patient safety. But not everyone thinks the FDA can keep up with this burgeoning new field. “The technology is evolving much faster than the regulations are,” says Dr Diamandis.

The amount of money flowing into digital-health start-ups is rising fast. Rock Health, a non-profit incubator in San Francisco that tracks deals in this field, says 128 venture-capital firms invested a total of $1.1 billion in more than 100 digital-health firms in the first nine months of 2012, an increase of 70% over the $626m invested in the same period in 2011. The number of deals rose by 84%.

Scanadu’s Scout

Handset-makers and wireless companies are also investing in mobile health and the development of mobile diagnostic devices. Nokia, a large handset-maker, has agreed to sponsor the $2.25m Sensing X Challenge, another competition being run by the X Prize Foundation that is intended to speed up the development of medical sensors for tricorders. And as well as financing the Tricorder X Prize competition, Qualcomm’s venture-capital arm has put money into several promising start-ups in the area, including San Francisco-based AliveCor, which has developed an iPhone case with two electrodes that can perform an electrocardiogram (ECG). Dr Topol recently used a prototype to assess a fellow passenger on an aircraft who was suffering from chest pain. He concluded the passenger was having a heart attack, and the plane was diverted.

Other firms are also developing medical add-ons for smartphones. MobiSante, based in Redmond, Washington, has devised a smartphone-based ultrasound system that was granted FDA clearance in early 2011. A hand-held ultrasonic probe plugs into a smartphone, which generates and displays an image. It costs $7,500, a fraction of the price of a conventional ultrasound.

Another smartphone attachment is the iBGStar, a sleek glucose-monitoring device for diabetics that plugs into an iPhone. Sold by Sanofi, a French drugs giant, it measures glucose levels in blood using a technology called WaveSense, developed by AgaMatrix, a firm based in Salem, New Hampshire. A test strip is put into the device and a drop of blood is applied from a tiny pinprick. The glucose level appears on a small display and is stored by an app on the iPhone, allowing for long-term monitoring. It was cleared by the FDA in December 2011.

These devices do specific things rather than being general-purpose devices. But Don Jones, head of business development at Qualcomm Life, argues that they could be regarded as specialised forms of a medical tricorder. Depending on need and location, he suggests, a tricorder’s design could be customised to perform different tasks. A good example is the CellScope, a project that originated in 2006 in Daniel Fletcher’s laboratory at the University of California, Berkeley, and has since spawned a start-up with the same name based in San Francisco. The CellScope is an attachment that turns a smartphone into a microscope. Several versions with different resolutions have been developed that allow the technology to be used for different things.

The academic group is developing versions of the CellScope that can be used for retinal scans or to detect pathogens, such as malaria and tuberculosis, by analysing images of slides smeared with samples of blood or sputum. Fifteen prototypes are currently being tested at clinics in Vietnam in collaboration with TB Reach, an initiative of the STOP TB Partnership, which is funding the project. Together with other academic partners, the group has developed software to automate the diagnosis of TB when no doctor is available.

MobiSante’s smartphone-based ultrasound

Meanwhile CellScope, the start-up, is designing optical attachments for the American market that turn smartphones into “digital first-aid kits”. The first is an otoscope (a device for looking inside the ear) that clips onto an iPhone. It could be used by doctors for diagnosis and visual record-keeping, or by parents to transmit images to paediatricians for the remote diagnosis of children’s ear infections, which cause millions of visits to the doctor each year. CellScope’s boss, Erik Douglas, says the firm is conducting a study with doctors from the Atlanta Pediatric Device Consortium to see how its device compares with standard digital otoscopes. He hopes it will go on sale next year for less than $200.

The firm that is making the most tricorder-like gizmo is arguably Scanadu, based in Mountain View, California, which was among the first companies to enter the Tricorder X Prize competition. Its device, like Dr McCoy’s, consists of a small hand-held sensor unit that communicates wirelessly with a display unit (in this case, a smartphone). The sensor unit, called Scout, is placed in contact with the patient’s temple and detects a range of vital signs, including heart and respiration rate, blood oxygenation, pulse transit time and temperature. The current prototype includes electrodes to measure the electrical signals of the heart and an infra-red temperature sensor, among other things. A smartphone app displays and stores the data.

So far the only blockbuster medical device to have become commonplace in the home is the thermometer, says Scanadu’s boss, Walter de Brouwer. His aim is for the Scout, which is small enough to be taken anywhere, to achieve similar ubiquity. “The idea is to make the tricorder for the consumer,” he explains. He hopes to have the first version on sale by the end of 2013 for around $150.

At the same time, the company is developing disposable molecular tests for consumers, including the “ScanaFlu”, which will indicate the presence of strep throat, flu or other types of upper-respiratory infections. Patients gargle a special liquid and then spit into a cartridge containing test strips. A pattern of lines will appear to show which infections, if any, are present. The long-term aim is to bridge the gap between the Scout, which measures vital signs electronically, and the molecular tests which analyse samples. “We want to give patients more power to diagnose their own illnesses and keep track of their own health,” says Aaron Rowe, Scanadu’s research director.

The doctor’s diagnosis

Not everyone is excited about patients taking matters into their own hands. Health care is a very paternalistic industry, and “physicians don’t want patients to become independent and too empowered,” says Mr Wasden of PwC. “The medical community has always been very conservative,” says Yan Chow, director of innovation and advanced technology at Kaiser Permanente, a non-profit health-care provider. “It’s very hard to change things.”

Moreover, doctors may be reluctant to use data collected by patients. Instead of measuring vital signs at an annual check-up, they could find themselves being asked to examine huge data sets created by patients—raising the question of legal liability if something is missed. “The irony is that a doctor is more comfortable with the liability in a system that does not have rich data than in a system that does have rich data,” says Mr Wasden. Another difficulty is that electronic health records are not designed to allow for the inclusion of patient-generated data, says Dr Chow.

“As medicine becomes more of an information science, some tasks could be taken on by patients.”

Some of the new diagnostic tools may be financially threatening to doctors, especially in disciplines such as optometry, dermatology and paediatrics, says Dr Topol. Why would you visit a specialist, he asks, when a mobile device lets you test your eyes, diagnose skin lesions or determine whether your child has an ear infection? But as medicine becomes more of an information science, some mundane and simple tasks could be taken over by patients, which could free up doctors for more demanding problems, argues Mr Jones.

Whether for reasons of cost-saving, convenience, a love of gadgets or simple hypochondria, there will be demand for any device that turns a smartphone—already a portable library, communicator, map and entertainment device—into a health-monitoring and diagnostic tool as well. “If it’s not prevented by regulations,” asks Dr Diamandis, “who would not want to have a tricorder in their pocket?”

Прежде всего, я была очень польщена и сочла за честь участвовать в процессе оценки проектов Сколково M.D. Это первый шаг, который поможет не только идентифицировать технологии, продлевающие жизнь или улучшающие ее качество, но и начать процесс "воспитания" нового класса предпринимателей в медицине (не ученых, а именно предпринимателей).

Ваше мнение об Отборе концепции мобильного диагностического устройства (МДУ): своевременна ли поставленная задача, возможна ли ее реализация, какие аналогичные разработки уже существуют?

М.А. Мониторирование, но не "самодиагностика" скорее найдут отклик среди врачей и пациентов. Наши соотечественники, к сожалению, имеют тенденцию увлекаться самодиагностикой, что значительно осложняет прогноз их лечения или даже приводит к фатальным последствиям. Мониторирование имеет действительно широкий спектр применения в медицине и здравоохранении.

Насколько понятны были условия Конкурса?

М.А. Для судей все было достаточно понятно и после проведенного Отбора очевидно, что финалистам условия также были понятны. Одно из пожеланий с моей стороны – придание большего фокуса на начальном этапе в процессе формулирования условий к потенциальным конкурсантам. Также у меня сложилось впечатление, что конкурсанты пытались ответить на все существующие медицинские вопросы и диагностировать одной метдикой или одним прибором совершенно разные нозологии. Если можно было бы попросить их сфокусироваться на каком то конкретном применении и том, как оно будет реализовано – было бы легче оценить проекты и дать шанс прорваться действительно лучшим из них.

Необходимо ли МДУ обычному человеку? Для чего?

М.А. См мой ответ на вопрос №1.

Какие проблемные моменты Вы обнаружили в работах команд?

М.А. Понимание термина такое «команда» у наших инноваторов в медицине отсутствует и напоминает мне подход в НИИ или Университетах: с 10-15 научыми руководителями, "большими" именами, имеющими порой отдаленное отношение к процессу построения компании и коммерческой реализации проекта. Это не диссертация, а большинство проектов именно походило на диссертации.

Я бы также ограничивала конкурсантов в обьеме, один из них расписал свою концепцию на 91 страницу.

What’s the Point? The recent convergence of information technology, medicine, and healthcare will help to meet the global need to improve efficiency and access to healthcare.

Recent efforts to increase life expectancy and quality of life in Russia continue to focus on therapeutic strategies addressing specific conditions. But emphasis on disease prevention, ways to efficiently monitor chronic conditions and access to the most recent information resources are equally important for physician and for individuals (before they become patients) and their families.

SkolkovoMD has recently completed its first competition for the best Mobile Diagnostic Device (MDD). Thirteen finalists will have a chance to showcase their innovative devices on December 12, 2012.

By building further on the SkolkovoMD efforts, we would like to expand the horizons of digital health innovation beyond mobile devices and introduce entrepreneurs and investors to best practices and opportunities across the multi-faceted world of digital health.

The Goal of this conference is to explore opportunities in digital health by introducing innovative concepts from existing players as well as from newcomers to the healthcare space. In addition, we will identify specific markets within digital health that are most attractive to Russia’s innovative ecosystem.

Объявлены финалисты Отбора МДУ / The MDD Selection Finalists Announced

Оргкомитет Отбора Концепции Мобильного Диагностического Устройства (МДУ) извещает о завершении полуфинального этапа. В Экспертную панель полуфинального этапа входили члены Оргкомитета и Эксперты Фонда «Сколково», представлющие научные учреждения и предприятия в области здравоохранения и информационных технологий.

С точки зрения потребителя - задача была универсальной – создание мобильного диагностического устройства для самооценки потребителем состояния здоровья и постоянного мониторинга основных жизненных показателей. Многие команды использовали похожие технологии и методики при описании предложенного решения для создания концепции. Но следует отметить, что некоторые из представленных концепций решают поставленную Фондом задачу лишь частично, предлагая устройства, которые хорошо диагностируют всего одно-два заболевания.

Всеми участниками, без исключения, была проделана важная работа, которая, по мнению Оргкомитета, не должна остаться незамеченной. На финальное мероприятие, которое пройдет в центре «Digital October» 12.12.12, приглашаются все сорок с лишним участников полуфинального этапа. При этом, у всех желающих участников будет возможность представить свои проекты зрителям финала и членам жюри.

Мы приветствуем новые заявки на статус участника Сколково от проектов и команд, которые были участниками полуфинала Отбора Сколково!

The Organizing committee and Selection committee of the Mobile Diagnostic Device (MDD) selection is announcing the completion of the semi-finals stage of the competition. The expert panel of this stage of the selection included members of the Organizing committee and experts panel of the Skolkovo foundation, which represent research institutions and enterprises, which work in the area of healthcare and IT industries.

From consumers’ point of view the problem to solve by the selection participants was of a universal character. They had to come out with a mobile diagnostic device for customers’ health self-assessment and constant monitoring of vital health indicators. Many teams used similar technology and methods to describe the proposed solutions to create a concept. But it’s worth mentioning that some of the concepts solve the problem only partially, offering devices that are able to diagnosticate only one or two diseases.

All the participants, without any exception, have carried out a very important job, which won’t be ignored by the organizing committee. The final event, which will take place at the “Digital October” venue on 12.12.12, will be open to all forty-odd participants of the semifinal stage. At that, all participants will be able to present their projects to the audience and the jury of the finals.

We welcome new applications for residence membership of the “Skolkovo” project from teams that took part at the semi-finals of the Skolkovo MDD selection!

For the final stage of the selection the Organizing Committee recommends the following teams:

Добрый день!

На этапе подготовки документов, для участия в полуфинале отбора у нас возник вопрос, касающийся формы предоставления 3D модели устройства. Должна ли эта модель представлять собой модель, сделанную в 3dmax, либо это может быть .jpg изображение. Дайте пожалуйста свой комментарий на эту тему, а именно в каком виде желательней всего подготовить данную модель?

Подскажите пожалуйста необходимо ли включать информацию об участниках команды и информацию о ранее выполненных проектах в полуфинальную заявку на конкурс, так как данная информация уже была включена ранее в заявку на участие. Спасибо.

Об NDA для участников Отбора Skolkovo MD

Уважаемые коллеги,

Как известно, чем меньше бюрократии, тем лучшей! Поступило разъяснение юридической службы Фонда, суть которого сводится к тому, что ваше согласие регистрироваться и участвовать в Отборе означает согласие с Положением об Отборе и, как следствие, заключение NDA. Поэтому в письменном виде заключать отдельное соглашение не нужно.

На юридическом языке это называется конклюдентными действиями – согласие с правилами путем подачи Заявки. Если Заявка подана, NDA считается заключенным автоматически по форме приложения 3, и, значит, документ подписывать юридически не надо.

Обоснование: пункт 7.5 NDA – «Подача Заявки, указанной в пункте 1.1 настоящего Соглашения, означает принятие Участником условий настоящего Соглашения. С этого момента Соглашение считается заключенным на вышеуказанных условиях».

Пункт 14.1 – «Фонд принимает на себя обязательство о неразглашении представленной Участником информации, указанной в его Заявке, для чего заключает с ним Соглашение о неразглашении представленной информации по форме, приведенной в Приложении 3 к настоящему Положению. Подача Заявки означает принятие Участником условий такого Соглашения о неразглашении представленной информации.»
